WAIVER OF LIABILITY FOR ALCOHOL CONSUMPTION, PHYSICAL ACTIVITIES, AND PHOTO PERMISSIONS

I acknowledge that alcoholic beverages will be available for purchase at the event hosted by the Beta Alpha Sigma Zeta Chapter of Zeta Phi Beta Sorority, Incorporated. I understand the risks associated with alcohol consumption and assume full responsibility for any loss, damage, or injury that may occur.

In addition, I acknowledge that physical activities may be part of the event. I understand the risks associated with participating in physical activities and assume full responsibility for any loss, damage, or injury that may occur as a result of my participation.

I also grant permission to the Beta Alpha Sigma Zeta Chapter of Zeta Phi Beta Sorority, Incorporated, to use photographs and/or video recordings including images of me both internally and externally to promote the chapter’s events. These images may be used in print and digital media formats including print publications, websites, e-marketing, posters, banners, advertising, film, and social media.

In consideration of participating in this event, I release and discharge the Beta Alpha Sigma Zeta chapter, their officers, employees, agents, and sponsors from any liability arising from my participation. I agree to indemnify and hold them harmless from any loss or costs incurred due to my participation.

I will monitor my alcohol consumption and ensure my safety. I will not drive under the influence and will arrange safe transportation. The chapter reserves the right to deny entry or remove individuals deemed intoxicated or posing a risk.

I have read and understood this waiver and release, and I voluntarily give up substantial legal rights by checking the agree box.
