Prior to the rental event I will share this paragraph with all other members of my rental party and confirm their voluntary assumption of all risks related to exposure to COVID-19 for themselves and those in their care.
1. I understand that the risk of exposure to COVID-19 exists in any setting where other individuals are present. According to the Centers for Disease Control and Prevention, COVID-19 is a highly transmittable disease that can result in illness or death, especially in vulnerable populations such as the elderly and those with underlying medical conditions. I am voluntarily assuming all risks related to exposure to COVID-19 for myself and those in my care.
2. Dance Step and all others attending the rental have attested that they:
i) understand that the risk of exposure to COVID-19 exists in any setting where other individuals are present. According to the Centers for Disease Control and Prevention, COVID-19 is a highly transmittable disease that can result in illness or death, especially in vulnerable populations such as the elderly and those with underlying medical conditions. Each adult attending is voluntarily assuming all risks related to exposure to COVID-19.
ii) understand that local health officials and the Centers for Disease Control and Prevention recommend that social distancing should be practiced by and between unvaccinated individuals unless the individuals are members of the same household.
3. I will agree that all unvaccinated attendees that are age 2 and over in my group will wear a mask in common areas at the theatre.
